The Neonatal Intensive Care Unit (NICU) Great Results After Discharge (GRAD) Program is for infants who have been discharged from one of our Yale New Haven Health NICU. The mission of the NICU GRAD Program is to provide family-centered, post-discharge medical and developmental care to support high-risk infants and young children during and after their transition from the hospital to home. Children in the NICU GRAD Program will be evaluated by a neonatologist, nutritionist and physical or occupational therapist. The therapist will perform an assessment to evaluate if the child is meeting their expected milestones. Your rehabilitation team will ensure the proper referrals are in place, so your child can receive required interventions from the appropriate PT, OT and/or SLP. When the infant turns 1-year-old, they are seen at Yale New Haven Hospital’s NICU GRAD Program for a detailed developmental assessment.
